• No provider-patient relationship by website use alone: Using the Service or submitting an online request does not create a dentist–patient relationship unless and until you are accepted as a patient and receive care.

• Emergencies: Do not use the Service for medical emergencies. If you believe you have a medical or dental emergency, call 911 or go to the nearest emergency room. For urgent dental issues, call our office directly.

 

Appointment Requests and Communications

If you submit an appointment request or form through the Service, we may contact you by phone, text, email, or mail using the information you provide. Appointment requests are not guaranteed until confirmed by our office.

Any dispute arising from or relating to these Terms or the Service will be brought in the state or federal courts located in Dallas County, Texas, and you consent to the jurisdiction and venue of those courts.

U.S. Legal Compliance

You represent that you are not located in a country subject to U.S. government embargo, and you are not on any U.S. government list of prohibited or restricted parties.
